Hub House
Our Hub House provides a welcoming, inclusive space where participants can connect, learn, and thrive. Designed as a central hub for activities, programs, and social engagement, the Hub House offers opportunities to build friendships, develop life skills, participate in group activities, and access supports in a comfortable and supportive environment. It is a place where participants can enjoy meaningful experiences, increase their independence, and be actively involved in their community.
Life Stage Transition Support
Supporting you through important life changes with confidence and independence. Whether transitioning from school to adulthood, moving into independent living, starting employment, or navigating changes in supports and services, our team provides practical guidance and tailored assistance every step of the way. We focus on building skills, increasing independence, and helping participants achieve their goals while making each transition as smooth and positive as possible.
Day Programs and Group Activities
Day, Holiday and Youth Programs are available to provide you with opportunities to build friendships, develop new skills, and enjoy meaningful experiences in a fun, inclusive environment. Whether it’s community outings, creative activities, life skills development or recreation, our programs are tailored to individual interests and goals. With a focus on promoting independence, confidence, social connection, and choice, we ensure every participant can enjoy activities that are engaging, rewarding, and person-centred.
Support Coordination (Level 2 and Level 3)
Our experienced Support Coordinators provide Level 2 Support Coordination and Level 3 Specialist Support Coordination to help you navigate the NDIS and make the most of your supports. We work alongside participants, families, and service providers to build capacity, strengthen support networks, overcome barriers, and achieve individual goals. With offices in Launceston and Burnie, our team is committed to delivering responsive, person-centred support tailored to each participant’s unique needs and circumstances.
Goal Setting and Capacity Building

Through person-centred planning and tailored capacity-building supports, we help you build independence, strengthen daily living skills, increase community participation, and work towards meaningful outcomes. Skill Development and Independence Building
We empower you to build the skills and confidence needed to live more independently and achieve your personal goals. Through tailored, person-centred supports, we assist with developing everyday life skills, decision-making, communication, budgeting, cooking, travel training, and community participation. Our focus is on fostering independence, increasing confidence, and helping each participant reach their full potential.
Household Assistance and Domestic Support
Practical household assistance and domestic support to help you maintain a safe, clean, and comfortable home environment. Our support workers can assist with tasks such as cleaning, laundry, meal preparation, shopping, and other household duties, tailored to individual needs and goals. We focus on promoting independence, building daily living skills, and supporting participants to live comfortably in their own homes.
Transport Services, including wheelchair-accessible transport
Safe, reliable, and flexible transport services to help you stay connected with your community and achieve your goals. Whether it’s attending appointments, social activities, work, education, or community events, our transport options include a dedicated 12-seater bus and wheelchair-accessible vehicles to meet a range of needs. We are committed to ensuring participants can travel comfortably, confidently, and independently.
Respite and Short-Term Accommodation
The Hub House includes two comfortable bedroom spaces available for short-term respite care, with one queen bed and two single beds, providing a safe, supportive, and enjoyable environment away from home. Whether it’s for a short break, building independence, developing new skills, or enjoying a change of scenery, our tailored supports ensure participants feel comfortable, valued, and engaged. We focus on creating positive experiences while providing families and carers with peace of mind and an opportunity to recharge.
Personal Care and Daily Living Assistance
Respectful, person-centred support to help you maintain your independence and live life your way. Our Personal Care and Daily Living Assistance services can include support with personal hygiene, dressing, meal preparation, medication prompts, household tasks, and daily routines. We work alongside each participant to build confidence, develop skills, and achieve their individual goals while ensuring they feel safe, comfortable, and supported.
In-Home Supports
Flexible, person-centred in-home supports designed to help you live comfortably and independently in your own homes. Our dedicated support workers can assist with daily tasks, household activities, meal preparation, personal care, and developing independent living skills. We work alongside each participant to ensure supports are tailored to their individual needs, goals, and preferences, empowering them to maintain their independence and enjoy a high quality of life.
Community Access and Social Participation
At Wilson Disability Care, we support participants to get out and about, connect with their community, and enjoy the activities that matter most to them.
